We are currently recruiting for a permanent part time Social Worker/Discharge planner.
Working as a member of the clinical team, the Social Worker/Discharge planner assists the provider (physician, NP), patient and family with the implementation and planning of the arrangements for the level of care, whether it be in another hospital, a care facility or in the patient’s home.
In this role, you would also support patient care delivery within the Diabetes program of the Nephrology department by supporting patients and families to understand the plan of care by coordinating care and facilitating choice so that care is individualized according to each patient’s needs.
As Social Worker/Discharge Planner, experience with individualized discharge care plans and in the dialysis discipline working with clients with diabetes will be considered strong assets. As the program is designated under the French Language Services Act, bilingualism will also be considered an asset.

This part time role is a daytime, Monday to Friday role with no call and no weekend work and offers enrolment in the Hospitals of Ontario Pension Plan (HOOPP), optional Health & Dental benefits, and progression through a competitive wage grid.
